Description

The goal of the Health Care for the Homeless (HCH) program is to provide health care for individuals and families who are currently struggling with homelessness in Manchester, New Hampshire.

HCH's mobile clinic provides primary medical care, mental health care, addiction counseling, social services, nursing case management, and health education. A multidisciplinary team of healthcare providers including physicians, family and psychiatric nurse practitioners, nurses, addiction counselors, and social work/mental health counselors staff the mobile clinic. HCH is in session every weekday at the largest shelter in New Hampshire and Tuesdays-Thursdays at a transition housing program for homeless families.

The goal of the Health Care for the Homeless program is to provide health care for individuals and families who are currently struggling with homelessness.

Results / Accomplishments

The Health Care for the Homeless program has succeeded in increasing access to health care for over 1,000 individuals and family members who are currently homeless in Manchester, NH.
